Appointment Cancellation and No-Show Policy Agreement

By scheduling an appointment with Viking Alternative Medicine, you agreed to the following terms regarding cancellations and no-shows:

  1. Notification Requirement: If you are unable to keep your scheduled appointment, please notify us at least 24 hours in advance. This courtesy allows us to provide care to another patient in need of treatment.
  2. No-Show Fee: If you fail to cancel or reschedule your appointment with at least 24 hours' notice, you will be charged a No-Show Fee of $25. This fee partially compensates for the time set aside for your scheduled care.
  3. Billing for No-Shows: A No-Show Fee invoice will be sent to the email address you provided within 24 hours of the missed appointment. This fee must be paid before any further appointments or services can be provided.
  4. Multiple No-Shows: If you miss three appointments without proper notification, we reserve the right to terminate our services to you. Regular attendance is crucial for effective treatment and to respect both our resources and other patients' needs.
  5. Accuracy of Contact Information: You are responsible for providing accurate contact details when completing our New Patient Intake Forms. These details are essential for receiving timely reminders and communications from us.
